For patients age 18 and over. Coverage begins on your enrollment date and is valid for 12 consecutive months.
1 comprehensive exam + 1 periodic exam (new patients); or 2 periodic exams per year (returning patients)
2 regular cleanings per year
Routine 2D X-rays as clinically necessary
Unlimited emergency exams (diagnostic evaluation and X-rays only; treatment not included)
Annual Wellness Intraoral Scan
Bi-annual Oral Cancer Screening
15% off all additional treatments
Child plan
